I AM AWARE OF:
1. the obligation to immediately inform the bailiff of any circumstances that affect the payment of maintenance allowance, i.e. termination of raising the child, commencement of performance of the maintenance obligation by the debtor, amendment of the court ruling about the allowance, commencement of enforcement proceedings in a foreign country, end of enforcement proceedings in a foreign country, ongoing enforcement proceedings in a foreign country, end of studies of the child;

2. the obligation to repay the maintenance allowance to the state if the maintenance allowance was paid irrespective of the fact that the conditions for receiving maintenance support stipulated by law were not met.

3. I agree that the application for maintenance support for the duration of enforcement proceedings will be passed on to the Social Insurance Board.
